Automation for Clinical Documentation: 12 ACS Case Tracker for Therapists

Written by:

This project supported a therapist managing high-conflict custody and reunification cases. The goal was to automate the processing of a specialized form: 12 Associated Clinical Signs (ACS) of Psychological Abuse—a detailed clinical intake used to assess psychological manipulation in co-parenting dynamics.

🧠 About the Form:

The 12 ACS form collects sensitive behavioral observations—such as whether a child has unjustifiably rejected a parent, uses adult-sounding rejection language, or references being “forced” into contact. Each response is rated on a frequency scale, making it a structured tool for clinical review and court reporting.

⚙️ Automation Workflow:

  1. Google Forms
Therapist completes the ACS assessment form for a specific child/client.
  2. Zapier Formatter
Automatically splits the child’s full name to assist in structured file naming.
  3. Google Drive Search
    Finds the existing client folder.
  4. Google Drive Search (Reunification Folder)
Identifies the correct subfolder dedicated to reunification-specific documentation.
  5. Google Docs
A formatted document is generated using the ACS data submitted—ideal for clinical notes or court-ready reports.
  6. Google Drive
Applies secure file sharing preferences to maintain confidentiality.

📈 Impact:

  • Streamlined therapist documentation for high-stress, high-stakes reunification cases
  • Reduced admin workload while maintaining consistency and HIPAA-conscious file handling
  • Improved readiness for court or case review with standardized language and rating structure
  • No more lost forms or disorganized notes—everything is auto-sorted and stored in the right place

This automation gives the therapist more time to focus on clinical insight and less time battling file chaos.
